The Mojave Desert holds many secrets and mysteries. One of them is the strange creatures and mutants, some of which are too sophisticated to be simply the product of radiation. "Old World Blues" explores the origins of these creatures, but the Courier will need to overcome numerous challenges before he can uncover the truth.

Midnight Science-Fiction Feature!
  1. After downloading and installing the add-on, load the game. You'll see a new quest among your active quests: "Midnight Science-Fiction Feature!"
  2. Head southeast from Ivanpah Race Track until you find the Mojave Drive-in, which is open and has a crashed satellite in front of the screen.
  3. Approach the large blue light on the back of the satellite. You'll have to wait for the movie to start, which will happen between midnight and 3 a.m.
  4. Once you're ready, activate the satellite and then examine it closely. After a bright blue flash blinds you, the first mission of the expansion will begin.

Welcome to the Big Empty
  1. After the introductory cutscene, you'll find yourself dressed in a patient's uniform on The Sink Balcony. Your first task will be to figure out where you are.
  2. Follow the balcony to the right until you find a vending machine. You can get a bottle of Sunset Sarsaparilla from it, and the metal crates nearby contain bottle caps, ammo, and other useful items.
  3. Return to the point where you started your journey and go through the door. Inside the room, you'll find many useful items, but you won't be able to use anything until you restore your memory.
  4. Explore the kitchen and bedroom to find food and crafting ingredients.
  5. Head to the room with two glowing elevator doors. The one on the right will take you to the Think Tank. Once you enter, you'll be able to use your weapons again, for reasons that remain unclear.
  6. Proceed forward and up the ramp. When you reach the top landing, you'll see a hologram of Doctor Klein and his assistants.
  7. Ask, "What is this place?" Dr. Klein will only mock you in response. Then inquire about the large monitor, which will allow you to meet Dr. Mobius.
  8. Once Klein and his assistants are done, inquire about your brain. This will unlock a new perk—Brainless—which will increase your resistance and damage tolerance. Also, if your Medicine skill is high enough, you can ask the robots to heal you.
  9. If you ask about the operation you underwent, you'll also receive the Heartless perk, which makes you immune to poison, increases the effectiveness of healing items, and reduces the accuracy of robotic enemies. You can also receive the Spineless perk, which increases your strength and damage cap.
  10. Ask what happened to your brain, and Dr. Klein will ask for your help in stopping Mobius. Agree, and you'll complete the current quest and be able to begin the next one.

Picking Your Brains
  1. Ask about what you need to find, then say you're ready to start right away. Ask for all the information you can about the pylons and offer to hand over your weapons—this will give you a Sonic Emitter.
  2. Talk and ask questions until Dr. Klein gets tired and turns away.
  3. Climb the stairs and speak with Dr. Klein again. This will activate the "Picking Your Brains" quest. You'll need to speak with all four robot assistants.
  4. As you interact with Dr. 8, Dr. Dala, Dr. Borows, and Dr. O, you will receive many new tasks.

He Came…And Went
  1. In conversation, ask Dr. Klein about the latest visitors.
  2. In the conversation, ask Dr. O about the attack on the Think Tank.

What's in a Name?
  1. Ask Dr. O his name. Asking the right question requires a certain amount of communication skills and intelligence.

Coming Out of Her Shell
  1. Ask Dr. Dahl why she looks at you that way during our conversation. You'll need a certain level of Perception to ask the right question.
  2. You can get a similar result if you give her a teddy bear.

All My Friends Have Off Switches
  1. Before starting the quest, explore the rooms on the upper level, collecting supplies and new clothes. You can also find a chemistry lab on the lower level.
  2. Head back down the shaft and go through the left elevator door past the vending machine into the room.
  3. Talk to the Sink Central Intelligence Unit to receive the Sonic Emitter (Revelation) upgrade. Ask any questions you can to learn as much information as possible.
  4. Use the SCI Unit to purchase necessary supplies and ammo or repair your equipment. Once you're ready, return to the elevator and proceed through the left door to Big MT.
  5. During this quest, you'll need to collect missing modules. They're located in three different labs on the same level, and you can collect them in any order.
  6. Following the mission marker on the map, head southeast and downward. Be alert and avoid Nightstalkers and Lobotomites. Continue in this direction until you reach the pipeline in the corner. Keep an eye on the area for traps and enemies.
  7. Cross the pipeline to reach the Y-17 Medical Facility and go through the door marked with a red star. Go through the room and through the next door to reach the main area.
  8. As soon as you get there, disable the robot doctor (Doctor Orderly) with the Sonic Emitter. Keep in mind that it will only be disabled for a few seconds, so quickly climb the stairs to the right.
  9. Neutralize or eliminate enemies until you reach the top. Be careful there—avoid the flamethrowers. Pick up the Auto Doc Enhancement from the chest on the right.
  10. Continue up to the next floor and destroy or neutralize the hostile robots again. In the table on the right, you'll find the Lair Recording - Auto-Doc (Sink Project: AutoDoc). Don't forget to also collect various supplies from the chests, tables, and lockers, and then head back downstairs.
  11. Use the Sonic Emitter to destroy the Doctor Orderly. Pick up another Auto-Doc Upgrade in the corner of the room.
  12. Exit the building the same way you entered. Head right along the broken fence to Boom Town. Watch out for the cyberdogs. After clearing the area, return to the Sink.
  13. Circle the Lair and, following the quest marker, head to the remaining two locations. If you're facing the elevator doors, you should move left around the section to avoid unwanted encounters.
  14. Continue in this direction until you reach the Magnetohydraulics Complex. Enter the flooded cave through the door in the wall. Go straight ahead and down the stairs until you see a room on the left.
  15. Go inside and grab another Sink Project recording from the table, as well as various supplies from the lockers and drawers. Incidentally, the local water restores your health.
  16. Leave the complex and head to the next marker on the map. As you move left along the wall, you'll soon see a tall metal staircase. Climb it and you'll reach the X-22 Botanical Garden. I recommend equipping a more powerful and accurate weapon—serious enemies await.
  17. You can find the next Lair entry on the right after you deal with the enemies.
  18. The next map marker will lead you to the Forbidden Zone Dome Entrance. Following the train cars and keeping left, you'll soon find the Big MT North Tunnel. Open the door and enter.
  19. Enter the room on the right and pick up the switch. Then exit the room and tunnel and head to the Y-17 Medical Facility.
  20. The next quest marker will take you to Higgs Village.
  21. Enter, go down the stairs, and enter the house to the southeast with a loudspeaker hanging on the door. Climb the stairs and you'll find yourself in a room filled with ancient electronics. Pick up a jukebox module in one room and an audio recording in the other.
  22. Leaving the village behind, head southwest to the next marker until you reach the X-2 Transmitter Antenna Array. Enter and explore the interior until you find a switch module near the coffee maker.
  23. Exit the antenna complex through the door on the third floor and head northwest. Follow the mountain until you find Cuckoo's Nest.
  24. Once inside, quickly move to the right before the enemies notice you. Between two skulls, you'll find the Lair entry: Toaster Module. Grab it and escape the cave.
  25. Return to Higgs Village and enter the fenced-in area. Go down to the lower floor and find door #101.
  26. Once inside, go up to the second floor and enter the room with the huge TV. Pick up the Book Chute part and Dr. Klein's glove.
  27. Leave the village and head to the Magnetohydraulics Complex. From there, follow the quest marker through the courtyard to the large white building. The markers on the map will help you find the Securitron De-Construction Plant.
  28. Jump onto the truck platform and pick up the Muggy module. Afterwards, with a sense of accomplishment, you can return to the Sink Balcony.
  29. Talk to and reactivate the Auto-Doc, Book Chute, Light Switches, Toaster, Jukebox, Biological Research Station, Sink, and finally, Muggy. This completes the quest.

Achievement / Reward: "Making Friends" – Reset all robots in the Lair.

X-2: Strange Transmissions
  1. Head to the X-2 Transmitter Antenna Array and enter the door.
  2. Go up to the third floor and even higher until you reach the satellite dish.
  3. You'll find an X-2 Transmitter Antenna in a metal box. This completes the quest.

A Brain's Best Friend
  1. During this quest, you will also complete the tasks “X-8: High School Horror!”, “Sonic Emitter Upgrade,” and “X-8 Data Retrieval Test.”
  2. Head to Higgs Village, enter the complex and go down the stairs until you reach the bottom floor.
  3. Using the marker on the map, find the doghouse near the rocket. At this point, a small monster called Stripe will attack you. It's generally best to avoid it rather than engage it. Your target is the dog bowl lying near the entrance to the doghouse. Once you've picked it up, simply run away from the area, and Stripe will leave.
  4. Head to Boom Town. The quest marker will point to the X-8 Research Facility. Enter and head down the slope until you see a glowing terminal on the wall.
  5. Activate it and select data recovery. Run the Basic Test. Once the test is complete, you can access the Testing Facility through the door on the right.
  6. Go through the door marked "Hall H." On your way through the hall, destroy the Hall Monitor on the left wall. Exit the room through the door at the far end.
  7. In the next room on the right, destroy the monitor as well. Then activate the terminal on the teacher's desk and obtain information about the students.
  8. Head down to the Library. Another terminal you can retrieve data from is in the corner.
  9. Continue through rooms and hallways, destroying enemies and monitors along the way, until you find another glowing monitor. A little further on, past it on the left, there's another door, and beyond that, a third data terminal.
  10. Having collected the necessary information, exit the room through the large door guarded by two cyberdogs.
  11. Activate the glowing terminal on the right and select three options simultaneously. This will add a new marker to the map.
  12. Exit and head to the room marked "Residential Observation." Continue through the hall to the next room, then activate the terminal in the corner. This will give you another task.
  13. Pick up the holotape lying near the terminal before leaving. Return the same way and exit through the door marked "Exit." Pick up another holotape from the table and activate the terminal on the right wall.
  14. Go through the large door on the right and activate the blue terminal in the corner to obtain the Pulse Wave Module. Now you can use the Sonic Emitter to disable the force field. Try it and complete the Sonic Emitter upgrade quest.
  15. Once all three student records are collected, you can leave the Testing Facility and return to the X-8 Central Facility. Activate the testing terminal again and run the Residential Cyberdog Guard Test.
  16. After the test, go through the large door on the right and eliminate Gabe. The Principal will then begin a countdown—you'll hear it over the speakers. To avoid the explosion, reenter the Central Facility before it reaches Unit 1 and use the terminal to reopen the door. When you return, you'll find the bomb has detonated in your absence.
  17. Go directly to the right of the entrance until you come across a pit and a hole in the fence. Pick up the audio sample; after a while, a message will appear informing you that the sample rate has been updated.

Achievement/Reward: Spinal-Tapped! – Find the X-8 frequency of the Spinal Impulse Desensitizer.
 
  1. Completing this quest will also complete the quest "X-8: High School Horror!"
  2. Return to the Central Facility. When you get the chance, fast travel to the Sink's Balcony and then head into the interior.
  3. Enter the Think Tank through the elevator door and speak with Dr. Borous. Give him Gabe's bowl. Note that completing this quest requires a certain level of communication skill.
