How little we know
Look for the sign that says "Brimstone." Look for Cachino, ask people around about him, and talk to him. To get the key to his room, go to the entrance and talk to the girl there about the key. Once you have the key, go to the elevator and go up to the rooms. Turn left twice and you'll find a room. Open it and go inside. Climb the stairs; there's a table in the corner; open it to find Cachino's journal. Return to Cachino. If you agree to keep quiet about Omertas' plans, he'll give you 100 caps.
Now you need to find Troike. You need to go down to the very bottom level. After exiting the elevator, walk past Goal and go through the door on the left. You'll find yourself in a hallway with doors. Open everything until you find Troike in a room with thugs. Troike will ask you to release him from his contract with Big Sal. There are three options here: with advanced conversation or barter, you can persuade Sal to cancel the contract.
With advanced medicine, you can say the story is fake, or you can simply search Sal's safe for evidence. Once you fulfill Troika's request, he'll agree to help you and tell you about the weapon. Now you need to give him time to destroy the weapon. Now return to Brimstone and talk to Cachino. He'll tell you that Big Sal and Nero's time is up and it's time to get rid of them. He'll give you the weapon in the conference room. Deal with Sal and Nero.
