It turns out to be a small teddy bear. Take it to the girl, and she'll give you a powerful magical amulet. If this quest isn't activated, the girl won't appear, and Demon Hork will leave behind some magical or unique item, like any other unique monster.

If the command.txt file contains the line "theoquest;", you'll see a little girl named Cilia near a tree on the way to Adria. She'll appear once you've visited the Hive. 

Talk to her, and she'll tell you that her only friend, Theodore, whom she used to play with, has disappeared. He was kidnapped by plants. You'll find him on the third level after you kill the Hork Demon.
